We are excited for you to start meeting with your therapist as soon as possible, so please complete these required intake forms before your scheduled first session. We may not be able to meet with you without those forms being fully completed, and will ask you to reschedule to another time to ensure all forms are completed. If any form you are filling out raises any questions or concerns, or if you need to cancel/reschedule your session please reply back to your therapist requesting this assistance.
