Request Appointment

This online form is a convenient way to schedule appointments. Cartessa Smith's appointments must be scheduled on the 1/2 hr (ie 3:30-4:30). The appointment must be approved by a staff member so same day appointments are not available on the portal. If you need a quicker appointment or if your appointment request is not approved within 24 hours, Rodney Dillon, our Practice Manager can be reached at 801-382-8758 Monday-Friday 8:00-4:00.
