Request Appointment

This online form is a convenient way to request a therapy or psychiatry appointment.

New Clients: You must complete all intake paperwork before your requested Intake Appointment can be approved and scheduled.
Once you create a TherapyPortal account and log in, you can see your six intake documents waiting there for you (in the Documents tab) to review and sign. When you complete those documents, your appointment will be scheduled and confirmed if it is still available. We operate on a first-come first-serve basis. The forms take about 10 minutes to complete.
