We will call you within one business day to discuss your appointment request. A welcome email will then be sent instructing you to complete your new patient paperwork online before your first appointment. The following completed forms are required the day before your appointment or your session will be canceled: Client Information, Consent Form, Privacy Policy and Insurance OR Payment Authorization. A credit card is required on file for all direct pay clients and for insurance plans that require copays or deductibles such as Medicare or Aetna. CenCal clients do not require a credit card on file.
