Request Appointment

This online form is a convenient way to schedule/reschedule appointments. Appointments are nonnegotiable meaning what you see for availability is what we have. Remember due to ensuring your same weekly/biweekly rotation, once a client with us, any cancelled appointments outside frequency parameters (see our FAQ page) will incur a $79 cancellation fee (non-negotiable) and within 48hrs can only be cancelled by directly emailing your clinician or texting during business hours. New clients: be sure to arrive 5 minutes before scheduled times with your ID, insurance card, and payment option (e.g., credit card) if you have not already completed your online intake paperwork. PLEASE NOTE: Most appointments are recurring meaning that if you are making an appointment for a child in the morning; this intake spot will likely be the only time available on a weekly or biweekly capacity.
