This online form is a convenient way to schedule appointments; however, please contact us by phone if you need any assistance. After completing your intake documents please Select the appointment time that best suits your needs. Once approved you will receive an email reminder 48 hours prior to your appointment and then a notification 24 hours prior to your appointment. If you can't keep your appointment please go into the portal and cancel or reschedule it. If you need a SAME DAY CANCELATION , please contact the office and leave a text message at 404-907-0431 no later than 9 am to avoid a $50.00 "no show fee"
PLEASE NOTE: If you are a new client, it is important that ALL the intake documents are complete prior to your session. If it is not completed 24 hours prior to your session your appointment will be cancelled and you will have the opportunity to reschedule once completed. Intake forms are a very important part of the process, so please complete them in their entirety.
ALL insurance information must be entered as well and the "responsibility for payment" form must be completed as well or you can risk cancellation. If you have questions about payment etc please email or text the office and someone will get back with you ASAP.
All accounts will show current balance, and you will be able to pay through the portal. Accounts with a balance of over $250 will not be able to schedule an appointment until balance is paid in full.
If at any time you lose your portal access just send a text and it will be reset for you.
