Request Appointment

This online form is a convenient way to schedule appointments; however, please note that this is a request only. We will follow up to confirm the appointment date and time. Regular sessions are 50 minutes and billed at a rate of $150 per session. Payment is due at the time of your session and is billed through our front office. We do not accept insurance but can provide a Superbill that you can submit to your health insurance if you have out of network benefits. Please let us know if you will be requesting a Superbill. Your counselor will e-mail you the necessary forms to fill out before your first appointment once the date and time have been confirmed.
