If you are a NEW client to our practice, in your appointment request comments, please include:
-what issue/topic you are wanting to address in counseling
-how you found our practice, and
-if you are planning to use insurance, please include what type of insurance you have. If you plan to use private pay, please indicate that.

Please remember that we require 48 hours notice to cancel an appointment, and we do charge the full session rate as the cancellation fee.
