Request Appointment

***Please Read the Below Instructions -

1. You MUST put your INSURANCE COMPANY NAME AND MEMBER ID NUMBER in the "Message to Practice" on the next page for approval.

( Please Note : If we don’t see Insurance info the appt will be AUTOMATICALLY DECLINED (If you do not have insurance, write "None") you will get an email asking you to call the office with the information. )

2. You can not request same-day appointments or appointments less than 56 HOURS in advance

3. NEW CLIENTS: If the appointment request is approved, "New client" paperwork will be sent to you via FormDR and MUST BE COMPLETED by the day before your appointment. This is so the insurance can be verified. It will be canceled and you will be given time to reschedule should this occur.

4. This appointment is a request, you will be contacted by email if the request is approved or denied. Keep an eye out for communication from Riverscape Counseling
