NEW CLIENTS: Once you request an appointment, our Intake Coordinator will get in contact with you to confirm the appointment time. Please note, you are REQUESTING an appointment, and it is not a confirmation of an appointment until our Intake Coordinator contacts you to confirm.
We are IN NETWORK with Anthem BC/BS, United Healthcare, Tricare, Aetna, and Cigna. We also participate with a few EAP programs. Network participation does not guarantee that services will be covered by your individual plan. Unless verified in advance you will be responsible for the full contract rate assigned by your insurance carrier (Varies from $65.00 - $150.00). Adjustments will be made after the EOB is received.
We are OUT OF NETWORK for all other insurances and will owe our out of pocket rate (up to $175.00) in full at the time of service. If requested we can send claims to your insurance on your behalf so any payment due by insurance will be sent directly to you for out of network reimbursement.
When requesting an appointment, please list your insurance type, group and ID number, and a few sentences on what you are seeking therapy for. This will quicken the appointment confirmation process.
