When requesting a new appointment, please include the following in your message:
Whether you will be a cash-pay client or plan to use insurance.
If using insurance, please provide:
The name of your insurance plan (e.g., Aetna, UnitedHealthcare, Cigna, etc.)
The name and date of birth of the primary subscriber, if it is not you.
Please ensure that all required documents are completed at least 48 hours prior to your appointment time. If paperwork is not completed within this timeframe, we may need to reschedule your appointment.
Insurance appointment requests are subject to insurance verification and completion of all required portal documents before the appointment can be approved and confirmed.
