Request Appointment

Welcome to LPP and we are so happy you are here with us! This client portal allows clients to conveniently request appointments and manage existing appointments. Please keep in mind that all appointment inquiries and/or changes are submitted as a REQUEST only and are not guaranteed appointments until CONFIRMED by our office. If your request for an appointment is approved, you will receive a confirmation email. If your request is declined, our office will follow up with you to find another appointment time that may meet your schedule. PLEASE NOTE that if a clinician is not listed below, this is because that clinician's schedule is currently full and therefore not accepting new appointments.
