New clients:
After your session request has been confirmed you will receive a Portal invitation via email. In the portal you will find Intake Forms to download and complete along with several documents to review and sign.
If you do not receive the portal invite, please check your junk mail folder and/or contact the office to re-send the portal invite.
Cancellation Policy: 24 hours notice is required for all cancellations or re-scheduled appointments to avoid a $100 late cancel fee.
Please have a seat and make yourself comfortable in the waiting room until you are called for your appointment.
