After scheduling, check your email for additional forms from Therapy Portal that need to be completed to book the appointment. There will be 6+ documents that require completion. Please check your junk folder if you do not see my email in your inbox. Using insurance is often considerably less costly than self-pay rates. The out-of-network/self-pay rate is $175 a session. We will provide a receipt for you to send to your insurance for out-of-network benefits should you make this request. We will send a Good Faith Estimate for your records. Please give at least 2 business days notice if you cannot keep your appointment to avoid a late cancellation fee of $75. Check your email for approval of your appointment request.
