1) Please complete ALL documents before your appointment. Your appointment start time may be delayed and the appointment time shortened, if paperwork has not been completed.
2) To make, change, or cancel an APPOINTMENT, use the Portal Calendar (late cancel charges apply when cancelled less than 48 hours ahead of time without exception). Our providers reserve this time for you and are paid from the revenue from this visit.
3)For billing, all information is in the portal including receipts. Make sure to download your receipts TIMELY and store them as they after no longer available after 3 months. Send a portal message to BILLING if you have other questions.
4) For CLINICAL questions/information SEND A MESSAGE IN THE PORTAL directly to your DOCTOR (responses usually the same business day but check with your DOCTOR for response times).
