Request Appointment

This online form is a convenient way to schedule therapy appointments and intakes for self-pay and insured potential clients. For ease of use, you can only view a week from today's date at a time. Please contact us by phone at 254-218-4161 or email at brittany@risecounselingandcoaching.com if you need any assistance OR if you would like to know what time slots are available for a date more than a week out.

Self-pay (you are not using insurance) rate: $100 per session.

**If you need a self-pay SLIDING SCALE/REDUCED RATE you will need to go to our Contact page on the website instead of requesting a session here. If you request an appointment through this screen you will be charged the full self-pay rate of $100/session if not using insurance.**

PLEASE NOTE: If you are a new client, you will need to e-sign and complete all documentation requested in your portal (sent over by the practice within 24 hours of your submitted appt request) PRIOR to your scheduled appointment time or your appointment will be canceled. It is recommended to not request a Therapy Intake appt any earlier than two days from today's date to accomodate for this paperwork to be sent to you and your portal account to be created.
