Request Appointment

SCHEDULING TESTING:
You will be able to view available dates for testing. Please note that testing sessions run from 8:00 AM to 3:00 PM, even if this is not reflected in the scheduling system initially. The displayed time slot is only a placeholder—your appointment is not confirmed until reviewed and finalized by our staff. Once you request a slot, you will be prompted to enter your name, date of birth, and contact information. Along with this PLEASE INCLUDE: YOUR INSURANCE NAME, MEMBER ID, AND GROUP NUMBER. After submitting your request, a staff member will contact you—via phone, encrypted email (IDENTILLECT), or secure portal messaging—to collect additional information and verify insurance benefits. Please monitor your spam folder for encrypted emails, as they are time-sensitive and may expire.

IMPORTANT: Requesting an appointment does not confirm your booking. The following steps must be completed before your date is secured:
• Verification of insurance benefits
• Signed consent forms
• Credit card information on file
• Completed new patient intake forms
Once these steps are complete, administrative staff will assign you a patient account number, which you will use to complete additional testing forms.

EXISTING PATIENTS:
Please DO NOT request new appointments through the open scheduling slots. Instead, contact our administrative team directly for assistance with rescheduling. You may use the portal to request cancellations or changes to existing appointments.
