I am only in the ASDI office on Wednesdays. Mondays are telehealth only. The rest of the week I am in my Westminster office.
NOTE to In-Office clients: If you have ANY upper respiratory symptoms on the day of your scheduled appointment please let me know so that we can switch to telehealth that day, or you can reschedule to another day when you no longer have symptoms. I reserve the right to ask that we meet via telehealth instead of in person at any time for any reason. Upper respiratory symptoms can be any or all of the following symptoms: fever, runny nose, cough, sore throat, feeling unwell.
Cancellations with less than 24 hours notice will be charged at the full out of network rates (not applicable for Medicaid/ Medical Assistance). Clients who miss 2 or more sessions without giving advance notice will be automatically terminated from treatment. THIS POLICY INCLUDES TELEHEALTH appointments.
New clients: be sure to arrive 20 minutes before scheduled times to complete paperwork if you have not brought your paperwork with you to the first session or completed it online. For Telehealth clients, please download, complete and upload ALL requested documents online via TherapyPortal. If you are unable to use the portal, completed documents can be scanned and sent via email to stephniet@stephthomas.com. If you have not returned completed documents in time you will need to reschedule your appointment until such time as all documentation has been received.
I am in-network with some insurance companies in my private practice, located in Westminster, MD. It is your responsibility to ascertain your benefits, copays, & deductibles.

ASDI is an out of network provider. ASDI clients are responsible for submitting statements (provided monthly) to insurance for potential reimbursement.
Payment/copays and deductibles are expected at the date of service. All clients will be asked to provide credit card information prior to beginning treatment. All clients are expected to keep up to date credit or debit card information, and to maintain funds so that copays are not declined. I reserve the right to charge an additional fee for declined charges.
