Effective January 1, 2025, all outstanding balances must be paid before scheduling new appointments and clients must have a payment authorization on file. In addition, late cancellation/no shows fees will increase. Payment Authorization forms and the Consent for Services form may be found in your client portal. Please take note that insurance plans may change at the beginning of each new year. Verify with your insurance company whether there are any changes (copays, deductibles, and co-insurances) to the services you currently receive. We hope to continue to serve you and provide quality healthcare throughout the current year.
