**NEW CLIENTS** YOUR APPOINTMENT REQUEST WILL NOT BE APPROVED UNTIL ALL OF YOUR INTAKE DOCUMENTS ARE COMPLETED AND WE HAVE RECEIVED YOUR PHOTO ID AND INSURANCE CARD. THERE WILL BE NO EXCEPTIONS FOR THIS. THE DOCUMENTS MUST BE COMPLETED AT LEAST 48 HOURS IN ADVANCE, SO SCHEDULE ACCORDINGLY TO GIVE YOURSELF ENOUGH TIME TO HAVE THEM ALL COMPLETED. IF THE FORMS ARE NOT COMPLETED WITHIN THE REQUESTED TIME FRAME, THERE WILL BE A $100 FEE CH ARGED. THERE WILL BE NO EXCEPTIONS TO THIS EITHER. THANK YOU SO MUCH!!! ๐Ÿ˜ƒ

FOR EXISTING CLIENTS, YOU WILL RECEIVE AN APPOINTMENT CONFIRMATION WITHIN 24-48 HOURS OF THE REQUEST.
