This online form is a convenient way to schedule appointments. However, paperwork for all new clients must be completed 48 hours before the scheduled appt. Failure to do so may result in an appt cancellation. If you are using insurance, it is your responsibility to be clear about your out of pocket expenses for the session and a credit card must remain on file to cover your out of pocket expenses. Your credit card will be processed before your session for the amount that you are responsible for so you may wish to speak to a billing representative before your initial session.
