PLEASE READ THESE IMPORTANT REMINDERS: 1) NEW & RETURNING PATIENTS: Driver's Licenses or similar form of ID will be required for ALL adults attending any appointment at each visit. This is necessary for clinic-wide security. Without these on file, your appointment may be rescheduled or transfered to telehealth. Telehealth services may not be covered by your insurance policy and this would become private rate. 2) NEW PATIENTS: Please log into this portal as soon as possible and complete all paperwork prior to the date of your appointment. If you arrive without completed paperwork, you must arrive 30 minutes early. If you do not arrive early and do not have completed paperwork, your appointment will be rescheduled and a cancellation fee will be charged. 3) Completing new patient paperwork as soon as possible allows us to provide you with BILLING ESTIMATES. Without this paperwork done well in advance of your appointment, we may not be able to communicate service costs to you prior to service. Verification of benefits with your insurance company can at times take hours, not minutes. 4) If you have forgotten your login information, please contact either your clinician or the main office (office@silverliningsclinic.com) to have it reset.
